The girls learn about the harsh side of modelling and are then tested on how well they can handle harsh criticisms. As one model must go through a difficult surgery, two lucky girls receive a surprise visit from someone from home.

Janice Dickinson returns along with Series 3 winner Eva Pigford on the set of a photo shoot for ElleGirl magazine and designer MarcEcko. Then Tyra directs the models in a black and white photo shoot where some of the girls burst into tears.
