In interviews accompanying investigation footage, the families of victims consider what could have been, survivors reflect on their great fortune and witnesses recollect the shock of the moment.

A simple journey goes horribly wrong and a family faces a tragic loss.

The Crash Investigation Unit faces an uphill battle to get the evidence they need from a driver who refuses to speak with them.

And a shocked family deals with the loss of a mother and wife during what should have been the joyous time of her life.

Investigators face a seven-car pile-up in a busy underground tunnel during peak hour.

A roadway used by more than 100,000 motorists each day is brought to a stand-still when a truck and six other cars are involved in a complex series of collisions.
